- If you prefer not to use insurance, we can provide you with expected session rates and a Good Faith Estimate of what services may cost. This can help you plan and prepare before care begins.

Accepted Insurance
We work with many major health insurance providers. If your insurance is not listed below, it may still cover services — especially if you have out-of-network benefits. We recommend checking your own insurance benefits or calling your insurer before your first appointment.
Common plans we accept include:
- Blue Cross Blue Shield of South Dakota
- Sanford Health Plan
- South Dakota Medicaid
- Other plans may be honored — contact us for an updated list or to verify your coverage.
Forms
To help streamline your first appointment and ensure we have important information ahead of time, please review the following documents. You are welcome to print and complete these prior to your visit.
Intake Form
This form gathers basic client information, contact details, and relevant background so we can begin your child’s care thoughtfully and efficiently.
Sliding Scale Request Form
If you would like to be considered for sliding scale fees, please complete this form. It helps us better understand your family’s circumstances and determine eligibility for reduced-rate services.
